City of Chicago Youth Continuum

The City of Chicago Youth Continuum aims to increase the awareness of the continuum of opportunities and interventions available to youth ages 0-24 in the City of Chicago. Youth-serving departments present pathways of referral in order to increase youth connection to support systems and share youth plans and data to map an ecosystem of opportunities across the city. This map of pathways will be distributed to caring adults–be it youth workers, community leaders, or parents–to equip them to give referrals to the proper resources and connect more youth to existing opportunities. These resources are available to access digitally via the My CHI. website and app. Opportunities are sorted by need level, type of opportunity, department, and age group.
